This form in used for registering new children on the first time attending James River Assembly. To help the registration process be faster just print, fill out and bring it with you to the check-in counter in the kids area at either location.
Check-in Authorization Form (89 kb)
This form is to add a person to your account for check-in and check-out. If a friend, relative or neighbor is bringing your child to church and you would like to give them permission to check-in and out your children just print, fill out and bring this to the check-in counter in the kids area.
